The Fort
Worth Clinic was founded over 50 years ago, since that time has
continued to provide quality health care service to Fort Worth area
residents. Today, our primary focus remains on meeting our patient’s
needs.
Our
multi-specialty practice enables us to provide a comprehensive range
of medical services including Internal Medicine, Rheumatology,
Neurology, Gastroenterology, Infectious Disease and General Surgery.
For your added convenience we also provide on-site Laboratory and
X-Ray facilities.
Our expert
team of physicians gives you access to the latest advancements in
medical knowledge and technology while holding fast to a traditional
practice philosophy. You will find familiar faces and a helpful
staff in a comfortable atmosphere each time you visit our clinic.
OTHER PROFESSIONAL
PROVIDERS
We utilize
the professional services of specially trained health providers
often referred to Physician Extenders. They are supervised by your
physician and
work in a collaborative effort with your doctor to assist you in
your health evaluation and treatment. They include Nurse
Practitioner, Physician Assistant,
Registered / Licensed Dietitian and Certified Diabetes Educator.
APPOINTMENTS
At the
Fort Worth Clinic, appointments are scheduled Monday through Friday
from 8:00 am - 5:00 pm. Please notify us of appointment cancellation
at least 24 hours in advance. If you have an urgent situation, your
call will be transferred to your doctor's nurse who will help you
arrange for immediate care.
BILLING & INSURANCE
On your
first visit to our clinic as a new patient, you will meet with a
patient services representative to establish a personal medical
file. We are a participating provider for many major health plans.
If your health care is covered by Medicare or a managed care plan
(HMO, PPO, etc), you will be asked to present your identification
card at each visit. Depending on the type of insurance you have, you
may be required to make a full payment or a co-payment each time you
visit the clinic. If your medical needs are connected with a
job-related injury or illness, please inform our patient
representative each time you visit the clinic for that injury or
illness.
As a part
of our service to you, we will file your insurance claim. If you
have any questions about your claim, please contact our insurance
department and we will be happy to assist you.
Any
balance that remains after insurance has paid becomes a personal
balance that you will be responsible to pay.
REFERRAL FORMS
Managed
care patients that need to see our specialists must provide a
referral form from their primary care physician prior to their
visit. Providing a referral form prior to your visit is required by
your health care plan. If you are on an HMO plan that requires prior
authorization to see any of our physicians please refer to your
plan's Managed Care guide for specific details involved.
