Do you
take my insurance?
We accept most all insurance plans available in the North Texas
area. Please call us for specific verifications. Among the accepted
plans are: Medicare, Multiplan, Blue Cross / Blue shield plans, Aetna, Cigna,
Humana, PHCS (Private Health Care Systems) , United Health Care,
UNICARE, First Health, Great West , One Health,
Beechstreet, Health Smart, ProNet, Texas Health Choice and others.
WE DO NOT ACCEPT SECURE HORIZONS.
Does my doctor see me if I need to go to the hospital?
Our internal medicine physicians, neurologists, surgeon, and GI
physician will see you the local hospitals. We utilize Harris
Methodist of Fort Worth, Baylor All Saints, Plaza Medical Center of
Fort Worth. Since the Doctors rotate
evening and weekend coverage you may be seeing a Fort Worth Clinic
physician other than the one you usually see for your primary care.
Communication between physicians is made for appropriate follow up
during your stay and after you are discharged from the hospital.
What hospitals do you admit to?
We utilize Harris Methodist of Fort Worth, Baylor All Saints, Plaza
Medical Center of Fort Worth.
Can I see my physician or do I see a Nurse Practitioner or Physician
Assistant?
Your doctor is very concerned with your continued health and
appointments are available Monday through Friday by calling our
Appointment desk. If you have an acute illness or episode most
likely, for your convenience, you can make a same day appointment
with our nurse practitioner or physician assistant. You still have
the choice of seeing your physician. A Physician is immediately
available to address serious conditions or concerns.

What are your office hours?
Appointments are available from 8:00 AM until 4:45 PM Monday through
Friday.
What if I’m sick after the clinic closed for the day or on weekends?
If you have a medical emergency call 911 or the Emergency Telephone
number in your area. Otherwise, you would simply call our answering
service at
817-249-3692. They will contact the “on-call” physician to follow up
with you.

909 Ninth Ave # 300, Fort Worth, Texas 76104. We are across Humboldt
Street from Plaza Medical Center in Fort Worth at approximately at
9th Avenue and Rosedale in the hospital district.

Will you see me if I do not have insurance?
Yes. You will be responsible for the services you receive today as
well as any laboratory tests or other services. We accept cash,
checks, VISA, MasterCard, Discover, and American Express credit cards. If you are
waiting for your insurance coverage to become effective we will
continue to verify coverage with your insurance company and
determine coverage. If they indeed pay subsequent to your visit we
will issue to you a prompt refund.
What if I’m running low or are out of my medicine?
It is important that you keep close count of your medications. You
should contact your local pharmacy a week before you run out. If you
run out and do not have medication, contact your pharmacy. The
pharmacy will contact us to handle your refill request. Our response
to your refill request will be handled the same day or the next day
in some cases.
Sometimes, your physician may deny a refill due to medical reasons.
Also, you may be requested to make an office visit so your health
and medication can be properly evaluated.
Can I see my medical record? Can I get a copy?
The clinic maintains your medical record for an extensive period.
You have authority to read it and receive a copy. If you are moving
or changing doctors due to insurance changes we make a copy
available free of charge to your new physician or to a specialist
that is following your care. You will need to sign a Medical Record
Release Form available by contacting us. Under HIPAA privacy laws you can request a time to look at your records
and write comments on a blank sheet of paper for inclusion into the
medical record. However, there are certain exceptions to the general
HIPAA previsions.
If you wish to personally obtain a copy there is a charge according
to Texas law.
